Your Privacy
1. We will not sell or give your name or any other information we receive from you or on your behalf to any other entity except as explicitly stated within this document.
2. Any information or data we gather during our engagement with you which uniquely identifies you will be held in strict confidence for use by our employees will be used only for the benefit of meeting our engagement agreement goals.
3. We will discuss your situation with the Federal Aviation Administration to the extent required to meet our engagement agreement goals. These discussions may be in person, by letter, or various electronic means as is appropriate at the time.
4. We will identify you by name, address, account number or other means to the medical vendors you specify to us as having information required to meet our engagement goals. In addition, a copy of the Request For Medical Information form, which you will have signed, will be provided to these same vendors to give them your permission to release information to us.
5. The records we gather and generate in meeting our obligation to you will be stored in our offices. These records will kept for a reasonable period of time, as determined from time to time by us, and then destroyed in a manner we deem to be appropriate to protect your privacy.
